Location & Hours

This is an in‑person position involving case management and performing home visits within the service area of Chittenden and Grand Isle counties. On‑call: Not typically, variable depending upon program needs.

Position Specific Requirements

• 2 years’ experience in either home health, home hospice, or med‑surg (or a combination of these three settings).
