As a vital member of our interdisciplinary team, you’ll deliver skilled nursing care, educate patients and families, and create a supportive environment that honors their wishes.
This is an in-person position involving case management and performing home visits within our service area of Chittenden and Grand Isle counties.
- 2 years' experience in either home health, home hospice, or med surg - or a combination of these three settings.
On-call: Not typically, variable dependent upon program needs.
