UVM Health - Home Health & Hospice is seeking a community hospice admission nurse who will be the first point of contact for clients entering hospice, coordinating admissions with intake, reviewing referrals for eligibility, and scheduling in-person admissions.
The role requires an active Vermont RN license, at least four years in hospice or five years in home health, and offers a $10,000 sign-on bonus for full-time external applicants, plus mileage reimbursement and relocation assistance.

We are looking for a community hospice admission nurse who will be the first point of contact for our clients and families as they enter onto hospice and will ensure that the experience is personal and individualized while honoring specific client goals. The admission RN will provide comprehensive coordination of admissions beginning with collaboration with our intake department, reviewing referrals for eligibility and then will schedule and complete in-person admissions. You will collaborate with the client and caregivers around hospice plan, while organizing and directing the initiation of hospice care along with the associated charting and care coordination before handing off to the RN case manager and assigned interdisciplinary team members.
