Job summary

Baptist Health in Florida is seeking an experienced RN Case Manager to support physicians and the interdisciplinary team in coordinating patient care, improving outcomes, and managing costs. The role covers facilitation of care across the continuum and discharge planning in collaboration with UR RN and Social Work Case Manager.

Minimum 3 years of hospital clinical experience with 1–3 years in case management is preferred; BSN within 3 years of entry date is required for non-leadership roles.

Qualifications

• Associates degree required.
• RN license required.

Responsibilities

• Facilitate collaborative management of patient care across the continuum.
• Coordinate communication with healthcare teams, patients, families and post-acute care services.
• Lead discharge planning and utilization management to ensure efficient care.
• Monitor readmission risk and implement improvement initiatives.
• Apply process improvement methods to evaluate outcomes.
