Job summary

V-Tech Solutions is seeking an experienced Registered Nurse / Case Manager to provide nursing, case management, and care coordination services to individuals with intellectual and developmental disabilities.

The RN / Case Manager will support individuals living in residential programs across Washington, DC, Montgomery County, and Prince George’s County, with hybrid work options and travel between locations.

Qualifications

• Minimum 3 years of Registered Nurse experience.
• Associate degree or equivalent in Nursing.
• Active RN license in Maryland and the District of Columbia.
• Delegating Nurse authorization in Maryland and DC, or willingness to complete required orientation.
• Experience as a Case Manager or Care Manager preferred.
• Experience working with individuals with intellectual and developmental disabilities is strongly preferred.
• Valid driver's license and reliable personal vehicle required.

Responsibilities

• Conduct nursing assessments and develop individualized care plans.
• Coordinate medications, treatments, appointments, immunizations, and other healthcare needs.
• Supervise clinical functions of LPNs, CMTs, and TMEs.
• Maintain accurate health records and Health Passports.
• Communicate with individuals, families, physicians, and healthcare providers.
• Provide clinical guidance and health-related training to program staff.
• Participate in ISP, PCP, and interdisciplinary team meetings.
• Provide developmental assessments and coordinate community healthcare resources.
• Promote health, safety, independence, and continuity of care.

Compensation

Compensation $86,000–$88,500 annually

Mileage reimbursement provided.
