Job summary

Dove Healthcare is seeking a Nurse Case Manager to coordinate, plan, implement, and evaluate nursing services for residents across our Wisconsin communities. In this role you will supervise day-to-day activities, review care plans for compliance, participate in on-call rotations, and help create a safe, home-like environment for residents and their families.

• Degree from an accredited school of nursing.
• Previous nursing experience.

Responsibilities

• Supervise the day-to-day activities of assigned residents and employees.
• Coordinate, plan, implement, and evaluate the nursing services provided to each resident using clinical competence.
• Review and audit compliance with the patient's plan of care and changing needs.
• Provide a safe, comfortable, and home-like environment.
• Participate in an on-call rotation.
