Job summary

Elara Caring is seeking a Hospice Registered Nurse Case Manager in Tinley Park, IL. This full-time role offers the chance to impact end-of-life care by coordinating patient needs, education, and bereavement support.

The ideal candidate holds a State RN license, a nursing degree, and at least 1 year of clinical nursing experience. Travel between patient homes is required; driving license or reliable transport is essential. Hospice/home health experience is preferred.

Responsibilities

• Assess hospice patients and families to identify physical, psychological, social, spiritual, and environmental needs.
• Monitor patient condition and communicate significant changes to the physician and care team.
• Educate and support patients, families, caregivers, and facility staff in the interdisciplinary Plan of Care.
• Provide education on hospice philosophy, pain and symptom management to staff in various care settings.
• Provide compassionate support at the time of death and complete bereavement assessments for families.

As a Hospice Registered Nurse Case Manager, you’ll contribute to our success in the following ways:

• Assess hospice patients and families to identify physical, psychological, social, spiritual, and environmental needs.
• Monitor patient condition and promptly communicate significant changes, concerns, or safety issues to the physician and care team.
• Educate and support patients, families, caregivers, and facility staff in following the interdisciplinary Plan of Care.
• Provide education on hospice philosophy, pain management, and symptom management to staff in nursing homes, hospitals, assisted living facilities, and other care settings.
• Provide compassionate support at the time of death and complete bereavement assessments for families and caregivers.

What is Required?

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
• 1+ year of experience as a nurse in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in hospice or home health environment is preferred

The base pay for this position is$85K to $90K hourly, based on the company’s good faithestimate at the time of posting. Actual pay will be determined based on factors such as education, experience, skills, and internal equity.
