Elara Caring seeks a Registered Nurse Case Manager to help grow the home care field by delivering compassionate, high-quality care. You will assess, educate, and support patients and families across diverse care settings, coordinating care with physicians and the care team.

The role requires an active RN license, 1+ year clinical experience, and ability to travel to patient homes. We offer comprehensive benefits and a 401(k) with employer match.

Qualifications

• Graduate of an accredited nursing program (Certificate, Diploma, Associate or Baccalaureate).
• 1+ year of clinical nursing experience.

• Valid driver's license and reliable transportation for travel to patient homes.

Responsibilities

• Assess hospice patients and families for physical, psychological, social, spiritual and environmental needs.
• Monitor patient condition and report changes to physician and care team.
• Educate and support patients, families, caregivers and facilities on the Plan of Care.
• Provide hospice philosophy, pain and symptom management education to staff.
• Provide bereavement support and complete assessments for families and caregivers.

Job Description:

As a Registered Nurse Case Manager, you’ll contribute to our success in the following ways:

• Assess hospice patients and families to identify physical, psychological, social, spiritual, and environmental needs.
• Monitor patient condition and promptly communicate significant changes, concerns, or safety issues to the physician and care team.
• Educate and support patients, families, caregivers, and facility staff in following the interdisciplinary Plan of Care.
• Provide education on hospice philosophy, pain management, and symptom management to staff in nursing homes, hospitals, assisted living facilities, and other care settings.
• Provide compassionate support at the time of death and complete bereavement assessments for families and caregivers.

What is Required?

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
• 1+ year of experience as a nurse in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in hospice or home health environment is preferred
