Job summary

Dove Healthcare in St. Croix Falls is seeking an experienced registered nurse case manager to mentor and educate a team of caregivers while coordinating and overseeing patient care. You will work with staff, patients, families, and other professionals to ensure person-centered care.

Responsibilities include supervising daily activities, evaluating nursing services, ensuring compliance with care plans, and contributing to a safe home-like environment. This is a full-time role with on-call duties.

Qualifications

• Current licensure in the State of Wisconsin as a registered nurse (RN).
• Degree from an accredited school of nursing.

As a registered nurse case manager, you will:

• supervise the day to day activities of assigned residents and employees
• coordinate, plan, implement, and evaluate the nursing services provided to each resident using clinical competence
• review and audit compliance with the patient's plan of care and changing needs
• provide a safe, comfortable, and home-like environment
• participate in an on-call rotation
• this is a limited term position with the potential to turn into permanent employment.

Required Qualifications:

• current licensure in the State of Wisconsin as a registered nurse (RN)
• degree from an accredited school of nursing
• previous nursing experience
