Job summary

Dove Healthcare – St. Croix Falls is seeking a registered nurse case manager to mentor and educate a team while coordinating and overseeing patient care. This role focuses on coordinating nursing services, supervising staff, and ensuring compliance with care plans in a home-like environment.

The position is full-time with a sign-on bonus of $5,000. Eligible candidates hold a Wisconsin RN license, nursing degree, and have prior nursing experience.

• Previous nursing experience required.

Responsibilities

• Oversee daily activities of residents and staff.

As a registered nurse case manager, you will:

• supervise the day to day activities of assigned residents and employees
• coordinate, plan, implement, and evaluate the nursing services provided to each resident using clinical competence
• review and audit compliance with the patient's plan of care and changing needs

• participate in an on-call rotation

Required Qualifications:

• current licensure in the State of Wisconsin as a registered nurse (RN)
• degree from an accredited school of nursing
