Job summary

HCA Florida Lake Monroe Hospital in Sanford, FL is seeking an experienced RN Case Manager to support patients and families through the care journey, coordinating services and developing care plans that reflect each patient’s needs.

You will partner with physicians, nurses, and department leaders to ensure safe transitions, clear communication, and high-quality, patient-centered care across the continuum.

Qualifications

• APRn or RN license required.
• Associate degree or RN diploma required.
• Acute hospital experience required.

Responsibilities

• Guide patients and families through orientation, explaining rehab philosophy, Medicare/insurance benefits, discharge criteria, and patient rights.
• Coordinate education for patients, families, and caregivers to encourage participation in treatment planning.
• Develop and implement individualized treatment plans reflecting patient strengths, needs and recovery goals.
• Perform psychosocial assessments and discharge planning, documenting findings and communicating needs to the care team.
• Collaborate with Rehab Program Director, Facility Case Management Director, and interdisciplinary team to ensure seamless patient-centered care.

Job Summary and Qualifications

By partnering with physicians, nurses, and department leaders, you will help ensure safe transitions, clear communication, and consistent quality across the continuum of care.

Your responsibilities will include:

• Guiding patients and families through program orientation, explaining the rehabilitation philosophy, Medicare and insurance benefits, discharge criteria, and patient rights
• Coordinating education for patients, families, and caregivers to encourage participation in treatment planning, goal discussions, and family conferences
• Developing and implementing individualized treatment plans that reflect the patient’s strengths, needs, and personal recovery goals
• Completing psychosocial assessments and discharge planning evaluations, clearly documenting findings and communicating needs across the care team
• Collaborating closely with the Rehab Program Director, Facility Case Management Director, and interdisciplinary team to ensure seamless, patient-centered care

What qualifications you will need:

• (APRN) Advanced Practice Registered Nurse, or (RN) Registered Nurse
• Associate Degree, or Registered Nurse Diploma
• Acute Hospital experience Required
