Job summary

Forthright CM is seeking a Per Diem Nurse Case Manager to join our field-based team in Portland, Oregon. You will coordinate care for injured workers, work with providers and insurers, and help drive return-to-work outcomes on a flexible schedule.

This role offers per diem engagement with mileage reimbursement and travel to medical appointments and job sites. A strong clinical background and independent work style are essential to deliver timely, patient-focused care.

Qualifications

• Active RN license required (California).
• Valid driver’s license.
• Car insurance.
• Strong clinical background (orthopedic, trauma, or case management preferred).
• Experience in workers' compensation or case management a plus.
• Excellent communication and organizational skills.
• Reliable transportation.

Responsibilities
