Forthright CM is seeking a Per Diem Nurse Case Manager to serve the Southern California region in a field-based role. You will coordinate care for injured workers, communicate with patients, providers, and insurers, and support return-to-work outcomes, ensuring effective care coordination on a flexible schedule.

This position offers mileage reimbursement, opportunities for advancement, and flexible work-from-home options, enabling a balance between field duties and remote coordination as needed.

Qualifikationen

• Active RN license in California.
• Valid driver's license and car insurance.
• Experience in workers’ compensation or case management preferred.
• Strong communication and organizational skills.

Rate: $45-52/hour + mileage reimbursement (productive/task time)
Location: Northern California Region (field-based)
Job Type: Per Diem / As Needed
