The Registered Nurse Case Manager is responsible for providing direct patient care, coordinating overall home care services, and documenting care for agency clients in accordance with Quality Standards under the guidance of a Clinical Supervisor. This role will primarily support Oswego County.

Home visits are conducted Monday-Friday, 8 a.m. to 4:30 p.m., with one weekend per month required. There is no on-call required.

Performance Responsibilities and Standards:

  1. Ensure quality and safe delivery of home health services, including case managing client caseloads, developing care plans based on assessments and OASIS evaluations, and ensuring documentation meets quality standards.
  2. Complete all OASIS evaluations; ensure documentation reflects nursing care and interactions, is clear and concise, and is completed timely.
  3. Coordinate with LPNs; provide guidance and education regarding patient care, working with the Clinical Supervisor and Patient Care Coordinator to manage patients using a team approach and appropriately utilizing LPNs.
  4. Supervise and oversee paraprofessional staff, including home health aides, and communicate relevant information to the department.
  5. Comply with all policies, procedures, and quality initiatives; maintain confidentiality; demonstrate knowledge of infection control, regulations, and compliance.
  6. Adhere to organizational quality improvement standards.
  7. Maintain and update clinical knowledge regularly.

Job Qualifications:

  1. Current New York State registered professional nurse license (BSN preferred).
  2. Valid driver’s license, auto insurance, and a vehicle for work (Fleet Program available).
  3. Self-directed, organized, able to work independently in a fast-paced environment.
  4. Experience with Hospice, End of Life, or Palliative Care is preferred.
  5. Proficient in computer skills, including electronic medical records software.
  6. Excellent communication skills.

Physical Requirements:

  1. Ability to sit or stand at a desk most of the day.
  2. Physical tasks including lifting, patient transfers, bending, and walking in various weather conditions.
  3. Ability to drive in all weather conditions.
  4. Sufficient visual and hearing ability for communication.
  5. Ability to use a telephone and computer.
  6. Bloodborne Pathogens Exposure Category III.

Salary and Benefits: Min-max annual rate: $72,415.71 - $83,922.76
