The Field Care Manager Nurse 2 assesses and evaluates members' needs to achieve and/or maintain optimal wellness by guiding members and families toward appropriate resources for care and wellbeing. The role involves varied work assignments requiring interpretation and independent decision-making.

The Nurse employs various strategies to manage physical, environmental, and psycho-social health issues, identifies and resolves barriers, and monitors patient progress through assessments. May create care plans and understands organizational strategies, making decisions with minimal guidance while following established procedures.

Required Qualifications

• Registered Nurse with 2 years of experience in home case/care management
• Experience working with adults
• Knowledge of community health and social service agencies
• Ability to travel within 30-40 miles
• Exceptional communication and interpersonal skills
• Ability to work independently within scope
• Proficiency with electronic medical records and MS Office
• Full-time availability (40 hours/week)
• Valid driver’s license, personal vehicle insurance (100/300/100 limits), and access to a reliable home internet connection (DSL or cable, no satellite)
• Must have a private home office space and meet TB screening requirements
• For Florida residents, ACHA Level II Background clearance required
• Passionate about improving consumer experiences

Preferred Qualifications

• BSN
• 3-5 years in home assessment and care coordination
• Experience in health promotion, coaching, wellness, managed care
• Bilingual in English and Spanish
• Case Management Certification
• Motivational Interviewing Certification or knowledge

Additional Information

• Work involves field visits, including at members’ homes and partner facilities
• Location: Sparta, WI, and surrounding areas
• Travel up to 40%
• Hours: Monday–Friday, 8:00 am – 4:30 pm CST
• This is a remote position within a specific geographic area.
• Internet must meet specified speed requirements; satellite is not permitted unless approved.
• Role involves patient-facing activities and TB screening.
• Salary range: $71,100 - $97,800/year, with potential bonuses based on performance.
