Job summary

Elara Caring is seeking a Hospice Registered Nurse Case Manager to coordinate patient care across home and facility settings in Michigan. You will assess needs, monitor changes, and guide families through the interdisciplinary Plan of Care.

The role emphasizes compassionate support at end of life, staff education on hospice philosophy and pain management, and collaboration with clinicians to deliver the Right Care at the Right Time.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited Nursing program (Certificate, Diploma, Associate, or Baccalaureate).
• 1+ year of experience as a nurse in a clinical care setting.
• Ability to lift up to 50 pounds and assist with patient transfers.
• Valid driver's license and reliable transportation to travel between patient homes.
• Hospice or home health experience preferred.

Responsibilities

• Assess hospice patients and families to identify needs across physical, psychological, social, spiritual and environmental aspects.
• Monitor patient condition and communicate changes to physician and care team.
• Educate and support patients, families, caregivers and facility staff in the Plan of Care.

• Provide compassionate support at time of death and complete bereavement assessments.

As a Registered Nurse Case Manager, you’ll contribute to our success in the following ways:

• Assess hospice patients and families to identify physical, psychological, social, spiritual, and environmental needs.

• Monitor patient condition and promptly communicate significant changes, concerns, or safety issues to the physician and care team.

• Educate and support patients, families, caregivers, and facility staff in following the interdisciplinary Plan of Care.

• Provide education on hospice philosophy, pain management, and symptom management to staff in nursing homes, hospitals, assisted living facilities, and other care settings.

• Provide compassionate support at the time of death and complete bereavement assessments for families and caregivers.

What is Required?

• Current State license as a Registered Nurse.

• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing

• 1+ year of experience as a nurse in a clinical care setting.

• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment

• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.

• Experience in hospice or home health environment is preferred
