Job summary

Beacon Hospice in Clarksville, TN seeks an experienced Registered Nurse Case Manager to join our hospice team. You will plan and coordinate patient care at home, ensuring compliance with laws and payer guidelines.

Responsibilities include comprehensive evaluations, care plan development, and accurate documentation while maintaining efficient visit routes and high-quality clinical service. Valid RN license, CPR, and reliable transportation are required.

Qualifications

• One (1) year of clinical RN experience.
• Current RN license in the state where services are rendered.
• Current CPR certification, driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation.

Responsibilities

• Create, implement, execute, and coordinate the overall plan of care to promote wellness and prevent re-hospitalization for patients in their homes.
• Plan and exchange information with physician, agency personnel, patient, family, and community resources to maintain continuity of care.
• Perform nursing evaluation visits, determine focus of care, and develop the plan of care within state guidelines.
• Ensure patients meet home health eligibility and medical necessity guidelines as defined by the payer.
• Maintain and submit accurate documentation and clinical notes including patient condition, goals, interventions, and responses.
• Establish an efficient visit route and provide high quality clinical services within the scope of practice defined by state laws.
