Job summary

Beacon Home Health in Paris, TN is seeking a Registered Nurse to join our Home Health team on a PRN basis. The RN will provide and direct nursing care to patients in their homes under physician orders and agency guidelines.

Responsibilities include evaluating patients, planning and implementing care, maintaining documentation, and coordinating with physicians and families to promote wellness and prevent hospital readmission. A valid TN RN license and CPR certification are required.

Qualifications

• 1 year of clinical RN experience.
• Current RN license in the state where services are rendered.
• Current CPR certification.

Responsibilities

• Create and coordinate the plan of care to promote wellness and prevent re-hospitalization.
• Conduct initial and comprehensive nursing evaluations and develop state-compliant plans of care.
• Maintain accurate documentation and clinical notes of patient condition and interventions.
• Provide high-quality nursing services within the scope of practice from admit to discharge.
• Communicate with physicians, agency staff, patients, families, and community resources.
