Job summary

Beacon Home Health in Clarksville, TN seeks a Clinical Manager (RN) to lead and supervise clinical services across home health patients. You will ensure quality care, drive performance, and coordinate with the Executive Director to align operations with standards.

You will conduct quality improvement activities, review data, and guide staff through case conferences, documentation, and productivity initiatives while fostering compassionate patient care and staff development in the community.

Qualifications

  • Bachelor’s degree in nursing with RN licensure in the applicable state.
  • Current CPR certification and valid driver’s license.
  • Minimum 2 years of clinical experience, including 1 year in a managerial role.
  • Ability to commute to multiple patient residences daily within service area.

Responsibilities

  • Lead and oversee clinical services, ensuring quality of care and regulatory compliance.
  • Participate in quarterly quality improvement meetings and implement action plans.
  • Collaborate with the Executive Director on improvements and corrective actions.
  • Oversee surveys/audits and track KPIs with operations leadership.
  • Conduct chart reviews, coordinate care, and supervise staff performance.

Job Type: Full-Time

Position Overview

The Clinical Manager (RN) is responsible for supporting and overseeing the delivery and quality of clinical services provided to all patients. This role will evaluate performance and make recommendations to increase productivity and improve the quality of care delivered. A few position highlights include:

  • Conduct/participate in quarterly quality improvement meetings, review pertinent available data, identify trends, and oversee the implementation of improvement plans.
  • Collaborate with the Executive Director on the implementation of quality improvement activities and corrective action plans.
  • Responsible for the oversight of and response to external and internal surveys and audits within the assigned area relating to the quality of care and clinically related KPIs in collaboration with operations leadership.
  • Complete documentation and chart reviews to identify variances in standardized care and care processes and to identify areas for improved performance related to patient care and organization.
  • Hold case conferences to ensure oversight of care, coordination of services and that standards are met.
  • Facilitate the development of performance improvement action plans, based on available data (e.g., survey results, QAPI and clinical record reviews).
  • Accomplish staff results by communicating job expectations; planning, monitoring, and appraising job results; coaching, counseling, and disciplining employees; developing, coordinating, and enforcing systems, policies, procedures, and productivity standards.

Provide services within the scope of practice as defined by the state laws governing the practice of nursing, initiates and coordinates the plan of care, collaborates with the patient/family, attending physician and other members of the IDG in providing patient and family care daily.

The Must Haves
  • Bachelor’s degree in nursing from an accredited school, preferred.
  • Current and unencumbered Registered Nurse (RN) license in the state where services are rendered.
  • Two (2) years of clinical experience, with at least one (1) year in a related management role.
  • Current CPR certification, driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
  • Must be able to reliably commute to multiple patient residences daily within the geographic area assigned.
