Job summary

Ascension in Cedar Park, TX is seeking a Registered Nurse to join our Case Management team. You will design patient transition plans, evaluate records, and coordinate care to ensure the right level of care post-discharge.

The role requires a nursing diploma and RN licensure, with preferred CCMC certification and BLS. Schedule is Days or PRN, with a supportive, mission-driven workplace focused on quality outcomes.

What Minimum Requirements You’ll Need

Licensure / Certification / Registration

• Required Credential(s):
  • Registered Nurse obtained prior to hire date or job transfer date.
• Preferred Credential(s):
  • BLS Provider. American Heart Association or American Red Cross accepted.
  • Certified Case Manager credentialed from the Commission for Case Manager Certification (CCMC).

Education

• Diploma from an accredited school/college of nursing required. OR Required professional licensure at time of hire.

Additional Preferences

• Experience with inpatient acute care case management preferred.
