Job description

The Registered Nurse - Case Manager is responsible for coordinating and managing patient care within an acute care facility. This role involves overseeing care coordination and discharge planning to ensure optimal patient outcomes. The Nurse will collaborate with interdisciplinary teams, manage patient caseloads, and maintain compliance with clinical standards. A minimum of two years of acute medical care management experience is required, along with an active California RN license.

Special Considerations
  • Weekend work typically required one full weekend per four weeks, subject to department needs.
  • Floating to other units is not required.
  • First-time travel nurses accepted but must undergo screening.
  • Time off restrictions apply; no more than 7 days of requested time off allowed.
  • Dress code requires business attire or scrubs.
Scheduling
  • Day shift with patient ratios approximately 1:25.
  • Schedule includes typically one weekend per month.
  • Shift assignments based on department needs and staffing requirements.
