Job Summary
The Registered Nurse - Case Manager provides skilled nursing care with a focus on acute care case management. This role involves patient assessments, medication administration, treatment planning, and collaboration with interdisciplinary teams to ensure comprehensive care. The nurse will support patients through clinical care and care coordination, requiring at least two years of relevant experience. Strong communication and teamwork skills are essential, and the position may involve floating between similar units as needed.
Responsibilities
- Conduct patient assessments and develop individualized treatment plans.
- Administer medications and monitor patient responses.
- Coordinate care across interdisciplinary teams including physicians, therapists, and social workers.
- Manage case documentation and charting using the Epic system.
- Provide patient education and support discharge planning.
- Float between similar units based on staffing needs.
- Ensure compliance with clinical protocols and safety standards.
Required Skills & Certifications
- Active Registered Nurse license in the state of California.
- Minimum of 2 years acute care nursing experience.
- Proficiency in clinical assessment, medication administration, and treatment planning.
- Experience with electronic medical records, preferably Epic.
- Excellent communication and interpersonal skills.
- Bachelor's degree in Nursing or related field.
- Valid driver's license, car insurance, and reliable automobile for travel if required.

- Case management experience in an acute care setting.
- Experience working in a fast-paced hospital environment.
- Familiarity with interdisciplinary team collaboration.
- Previous experience with patient discharge planning and care coordination.
Special Considerations
- No on-call requirements.
- Floating between similar units may be required.
- Weekend work every other weekend (EOW).
- Position located in Eureka, California, near multiple state and national parks and the Pacific Ocean.

Scheduling
- Work schedule includes every other weekend shifts.
