Job summary

Ascension Health in Austin, TX, is seeking a dedicated Registered Nurse for Case Management at Dell Seton Medical Center. This full-time position supports the Case Management department with day shift hours (8:00 AM–5:00 PM, Monday–Friday) and a 40-hour workweek.

The role focuses on coordinating care across the continuum, advocating for resources, and ensuring accurate medical documentation post-discharge. Licensure in Texas and BLS eligibility are required or preferred, with CCMC credentialing

Qualifications

  • Licensed Registered Nurse credentialed from the Texas Board of Nursing is required.
  • BLS Provider preferred.
  • Case Manager credentialed from CCMC preferred.

Responsibilities

  • Create plan for care across the continuum, integrating patient/family preferences and values.
  • Monitor patient care through assessments, evaluations and/or patient records.
  • Advocate for resources and removal of barriers.
  • Maintain ongoing dialog with supervisor and other health providers to ensure effective implementation of health plan.
  • Act as a resource for adequate medical record documentation, appropriateness of services as they relate to diagnoses, and treatment options for post-discharge care.

What minimum requirements you’ll need
Licensure / Certification / Registration:
  • Licensed Registered Nurse credentialed from the Texas Board of Nursing required.

  • BLS Provider preferred. American Heart Association or American Red Cross accepted.

  • Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.

Education:
  • Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
