Job summary

Harborview Medical Center, part of UW Medicine, invites an experienced Registered Nurse to join the Care Management Team in Seattle. This 80% FTE, 4-day-a-week role is on the day shift, focusing on coordinating inpatient care and transitions for patients on acute care units.

The ideal candidate has inpatient trauma/general surgery or acute care experience, and a WA RN license with BLS certification. Harborview is a Level I Trauma center with comprehensive care services.

Qualifications

  • Two years of recent inpatient nursing experience in Trauma/General Surgery, Critical and/or Acute Care, Emergency Care, or equivalent experience
  • One year of Case Management or discharge planning experience and understanding of care coordination and transitions of care
  • Current license to practice as a registered nurse in the State of Washington
  • Current BLS certification

Responsibilities

  • Serve as a liaison between patients, families, medical teams, nursing staff, social services and referring physicians

Harborview Medical Center has an outstanding opportunity for an experienced Registered Nurse to join our Care Management Team.

WORK SCHEDULE:
  • 80% FTE (4 days a week)

  • Day shift

DEPARTMENT DESCRIPTION:

This Care Management team coordinates all aspects of in-patient care leading to a timely transition to the next level of care for patients on acute care units. They also serve as a liaison between patients, families, medical teams, nursing staff, social services, interdisciplinary team members, consulting and referring physicians and agencies involved in the care of patients from admission through the discharge process.

REQUIREMENTS:
  • Two years of recent inpatient nursing experience in Trauma/General Surgery, Critical and/or Acute Care, Emergency Care, or equivalent experience

  • One year of Case Management or discharge planning experience and understanding of care coordination and transitions of care

  • Current license to practice as a registered nurse in the State of Washington

  • Current BLS certification

Compensation, Benefits and Position Details
Pay Range Minimum:

$46.97 hourly

Pay Range Maximum:

$87.02 hourly

Other Compensation:

BSN (or higher degree) Premium - $1.00/hour ; Certification (Specialty Certs) Premium - $1.25/hourOvertime Pay$5000 Relocation Assistance (for eligible new employees)
