UOVUNIUSREXTERNALENUS The Registered Nurse Care Coordinator (RNCC) is responsible for organizing patient and family-centered care activities among multiple participants. The RNCC uses advanced critical thinking to develop, implement, and evaluate individualized care plans to empower patients and optimize outcomes. They coordinate resources and care activities to support transition management across the continuum, facilitating follow-up and service coordination across healthcare settings.
This role aligns with the ANA Nursing: Scope and Standards of Practice, the ANA Code of Ethics for Nurses, the AAACN Scope & Standards of Practice for Professional Ambulatory Care Nursing, and the UVA Nursing Professional Practice Model.

Position Compensation Range: $85,820.80 - $113,401.60 annually.
Minimum Requirements:
Physical Demands: Standing for prolonged periods, frequent travel, bending/stooping, communication skills, ability to lift/push/pull 20-50 lbs, potential exposure to chemicals and biological hazards.
