Job description

UOVUNIUSREXTERNALENUS The Registered Nurse Care Coordinator (RNCC) is responsible for organizing patient and family-centered care activities among multiple participants. The RNCC uses advanced critical thinking to develop, implement, and evaluate individualized care plans to empower patients and optimize outcomes. They coordinate resources and care activities to support transition management across the continuum, facilitating follow-up and service coordination across healthcare settings.

This role aligns with the ANA Nursing: Scope and Standards of Practice, the ANA Code of Ethics for Nurses, the AAACN Scope & Standards of Practice for Professional Ambulatory Care Nursing, and the UVA Nursing Professional Practice Model.

Core responsibilities include:

• Applying the UVA Professional Practice Model through nursing practice, quality achievement, lifelong learning, leadership, innovation, and caring.
• Planning, coordinating, and prioritizing patient care considering individual needs and outcomes, collaborating with the inter-professional team.
• Ensuring safety and continuity through documentation and hand-off tools.
• Advising patients, families, and caregivers, respecting their cultural values.
• Providing tailored health education and coaching based on evidence-based practices.
• Promoting health equity and leading interdisciplinary teams to improve population health, patient experience, and reduce costs.
• Applying research and evidence-based practices for continuous improvement across the lifespan and continuum.
• Facilitating care transitions, pre-visit coordination, post-clinic follow-up, and handoffs.
• Empowering patients with chronic illnesses through education on evidence-based standards and self-management strategies.
• Developing action plans based on patient needs and nursing assessment.
• Collaborating and communicating effectively to evaluate treatment outcomes and track progress.
• Supporting medication management.
• Performing other duties as assigned.

Position Compensation Range: $85,820.80 - $113,401.60 annually.

Minimum Requirements:

• BSN from an accredited program.
• At least 1 year of RN experience; 3 years preferred.
• Board Certification is strongly preferred.
• Virginia RN license and BLS certification required.

Physical Demands: Standing for prolonged periods, frequent travel, bending/stooping, communication skills, ability to lift/push/pull 20-50 lbs, potential exposure to chemicals and biological hazards.
