Role Overview

The Registered Nurse Care Coordinator (RNCC) organizes patient and family-centered care activities, using advanced critical thinking to develop, implement, and evaluate individualized care plans. The RNCC coordinates resources and activities to support transition management across healthcare services and locations, aligning with professional standards and models.

Minimum Requirements

• BSN from an accredited program.
• 1 year of RN experience required; 3 years preferred.
• Board Certification strongly preferred.
• Virginia RN license and BLS certification required.

Physical Demands

Prolonged standing, travel, bending/stooping, lifting 20-50 lbs., and exposure to chemicals, blood, and infectious diseases.
