UOVUNIUSREXTERNALENUS The Registered Nurse Care Coordinator (RNCC) is responsible for organizing patient and family-centered care activities among multiple participants. The RNCC applies advanced critical thinking to develop, implement, and evaluate individualized care plans to empower patients and optimize outcomes. They coordinate resources and care activities to support transition management through collaboration across the continuum, addressing patients' needs for follow-up and service coordination across healthcare settings.

This job profile aligns with the ANA Nursing: Scope and Standards of Practice, the ANA Code of Ethics for Nurses with Interpretive Statements, the AAACN Scope & Standards of Practice for Professional Ambulatory Care Nursing, and the UVA Nursing Professional Practice Model.

Care coordination and transition management require professional assessment, patient risk identification, and recognition of individual needs and preferences, including but not limited to:

  • Demonstrating the UVA Professional Practice Model through nursing practice, quality achievement, lifelong learning, leadership, innovation, and compassionate care.
  • Planning, coordinating, and prioritizing patient care considering unique needs and outcomes in collaboration with the interdisciplinary team, including recommendations and escalations as needed.
  • Ensuring safety and continuity of care via documentation, hand-off tools, and processes.
  • Collaborating with and advising patients, families, and caregivers in healthcare decisions, respecting their cultural values.
  • Providing health education and coaching tailored to issues within treatment plans, using evidence-based standards.
  • Promoting health equity through facilitative leadership across the continuum and teams to improve population health, patient experience, and reduce costs.
  • Applying research and evidence-based practices to improve patient care across the lifespan and continuum.
  • Leading efforts to ensure care continuity and consistency, including pre-visit coordination, follow-up, handoffs, and transition monitoring.
  • Educating patients and families with chronic illnesses on evidence-based self-management strategies.
  • Identifying support needs and developing action plans for patient-centered care planning and nursing process application.
  • Contributing to problem-solving through communication, collaboration, and evaluating treatment outcomes, including tracking progress toward goals.
  • Supporting medication management.
  • Performing other duties as assigned.

Position Compensation Range: $85,820.80 - $113,401.60 annually

MINIMUM REQUIREMENTS:

  • Education: Bachelor of Science in Nursing from an accredited program.
  • Experience: At least 1 year of RN experience required; 3 years preferred.
  • Board Certification: Strongly preferred.
  • License/Certification: RN licensure in Virginia; BLS certification required.

PHYSICAL DEMANDS: Standing for prolonged periods, frequent travel, bending/stooping, communication skills, ability to lift 20-50 lbs., and exposure to chemicals, blood, and infectious diseases.
