The Registered Nurse Care Coordinator (RNCC) is responsible for organizing patient and family-centered care activities among multiple participants. The RNCC applies advanced critical thinking to develop, implement, and evaluate individualized care plans aimed at empowering patients and optimizing health outcomes. The RNCC coordinates resources and care activities to support transition management through collaboration across the healthcare continuum, ensuring follow-up and service coordination tailored to patient and family needs.

Position Compensation Range: $85,820.80 - $113,401.60 annually.
