An established industry player is seeking a dedicated Registered Nurse Care Coordinator to enhance patient care through effective coordination and collaboration. This role will have 1-2 days of onsite work.
The Registered Nurse Care Coordinator (RNCC) is responsible for organizing patient and family-centered care activities among multiple participants. The RNCC uses advanced critical thinking to develop, implement, and evaluate individualized care plans to empower patients and optimize outcomes. The RNCC marshals and coordinates resources and patient care activities to support transition management through collaboration across the continuum, supporting patient and family needs for follow-up and coordination of services across healthcare settings.
This job profile aligns with the ANA Nursing: Scope and Standards of Practice, the ANA Code of Ethics for Nurses, the AAACN Scope & Standards of Practice for Professional Ambulatory Care Nursing, and the AAACN Scope and Standards of Practice for Registered Nurses in Care Coordination and Transition Management, integrated with the UVA Nursing Professional Practice Model.

UVA Nursing Professional Practice Model includes:
$85,820.80 - $113,401.60 annually
Standing for prolonged periods, frequent travel, bending/stooping; proficient communication, auditory, and visual skills; ability to lift/push/pull 20-50 lbs; exposure to chemicals, blood, body fluids, and infectious diseases.
