Independence First in Milwaukee is seeking a part-time Registered Nurse Care Coordinator to join a collaborative team and support consumers with personal care services. The position offers a hybrid in-office/remote work arrangement and a benefits package including paid time off and a 403(b) plan.

Responsibilities include supervising personal care workers, conducting home visits and assessments, and developing care plans to ensure quality, safety, and client satisfaction.

Qualifications

• Must be a graduate of an accredited school of nursing.
• Must have valid RN license where the assigned facility is located.
• Must have valid Driver License and good driving record.

• Part Time 28 hours per week M-F Hybrid (in office/remotework) schedule, No weekends, on call, or nights
• 18 vacation days and 12 paid holidays a year (prorated forpart time depending on hours worked)

Position Summary: Registered Nurse is responsible forthe supervision of personal care workers and the delivery of personalcare services to consumers. Work duties include home visits, consumerassessments, development of appropriate plans of care, and supervisionof and performance evaluation of personal care workers. Reports to theManager of Personal Assistance Services.

Experience: one year supervisory experience

Certificates and Licenses:

• Must be a graduate of an accredited school of nursing.
• Must have valid license as Registered Nurse where the assignedfacility is located.
• Must have valid Driver License and good driving record.
