Job summary

Live Well Homecare is seeking a Registered Nurse to join a mixed Cardiac/Tele and Neuro/Stroke unit, providing both bedside and virtual patient care.

You will monitor patient status, perform medication reconciliation, educate patients, and document interactions in EPIC, ensuring high-quality care and efficient discharge planning.

Qualifications

• Associate or higher degree in Nursing
• Illinois RN license in good standing
• CPR certification required
• ACLS and NIHSS certifications within 6 months of hire
• Bachelor’s degree or higher preferred; 1+ year inpatient nursing experience preferred
