Job summary

Piedmont Augusta Hospital in Georgia is seeking a dedicated Registered Nurse for Home Health to deliver patient‑centered care in patients' homes. You will assess needs, implement plans of care, and coordinate with physicians, therapists, and family members to support safe, high‑quality outcomes.

Qualifications

• Graduate from a recognized, accredited school of nursing Required
• No experience required Required
• RN - Registered Nurse - Georgia State Licensure and/or NLC/eNCL Multistate Licensure Upon Hire Required
• CPR - Cardiac Pulmonary Resuscitation Upon Hire Required
• DL NUMBER - Driver License, Valid and in State Current Drivers License, vehicle insurance, and access to a dependable vehicle or public transportation Upon Hire Required

Responsibilities

• Provides and directs provisions of nursing care to home health patients as prescribed by the physician.
• Coordinates total plan of care with other health care professionals involved in care.
• Maintains continuity of patient care in a Point of Care setting

Responsibilities

$10,000 Sign-On Bonus available

The Registered Nurse in Home Health provides and directs provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. The Registered Nurse coordinates total plan of care with other health care professionals involved in care and helps to achieve and maintain continuity of patient care by planning and exchanging information with physician, agency personnel, patient, family, and community resources. All done within a Point of Care setting.
