Job summary

Piedmont Augusta Hospital is seeking a Registered Nurse for Home Health to provide and coordinate nursing care to patients in their homes, following physician orders and agency policies. You will collaborate with physicians, other health care staff, and families to ensure continuity of care and positive patient outcomes.

• Graduate from an accredited nursing program.
• No prior experience required.
• RN licensure in Georgia or multi-state licensure.
• CPR certification required.

Responsibilities

• Provide and direct nursing care to home health patients as prescribed by the physician and in accordance with laws and agency policy.
• Coordinate total plan of care with other health care professionals and communicate with physician, patient, family, and resources.

The Registered Nurse in Home Health provides and directs provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. The Registered Nurse coordinates total plan of care with other health care professionals involved in care and helps to achieve and maintain continuity of patient care by planning and exchanging information with physician, agency personnel, patient, family, and community resources. All done within a Point of Care setting.
