Job summary

Piedmont HealthCare is seeking a Registered Nurse for Home Health to deliver compassionate nursing care to patients in their homes, under physician orders and in compliance with policies. You will coordinate care with other healthcare professionals to maintain continuity and quality of patient outcomes.

The role requires an RN license (Georgia or multi-state), CPR certification, a valid driver’s license, and reliable transportation.

Qualifications

• Graduate from an accredited school of nursing.
• No experience required.
• RN license in Georgia or multi-state licensure (NLC/eNCL) required on hire.
• CPR certification on hire required.
• Valid driver’s license and access to reliable transportation.

Responsibilities

• Provide and direct nursing care to home health patients as prescribed by a physician.
• Coordinate the total plan of care with other health professionals to ensure continuity of patient care.

Responsibilities:

The Registered Nurse in Home Health provides and directs provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. The Registered Nurse coordinates total plan of care with other health care professionals involved in care and helps to achieve and maintain continuity of patient care by planning and exchanging information with physician, agency personnel, patient, family, and community resources. All done within a Point of Care setting.
