Job summary

Lyngblomsten is seeking an RN for Assisted Living and Home Care at the Como Park campus in Minnesota. The role is full-time, non-exempt, with a Monday–Friday schedule from 8:30am to 5:00pm.

Pay range is $33.65–$40.48 per hour, with factors affecting the final offer and a focus on pay equity across the team. The position reports to the AL Clinical Director and includes care planning, collaboration with medical teams, staff supervision, and resident-family coordination.

Qualifications

• Current, unencumbered MN RN license.
• Minimum of two years direct nursing experience in long-term care, Home Health, or Assisted Living preferred.
• Excellent communication skills; Eldermark experience a plus.

Responsibilities

• Conduct resident assessments per policy and regulations (pre-admission, admission, 14-day, quarterly, and changes in condition).
• Supervise unlicensed staff and develop care plans.
• Coordinate with physicians, laboratory, hospice and other providers; assist with care conferences as needed.
• Provide back-up to after-hours triage service.
