Responsibilities

• Lead and support Resident Care Assistants to ensure excellent resident care.
• Complete resident assessments and individualized service plans with residents, families and the care team.
• Monitor resident health, respond to changes in condition, and arrange follow-up care.
• Oversee medication management, physician orders and clinical documentation.
• Coordinate appointments and other resident wellness needs.
• Support resident transitions and ensure clear communication and documentation.
• Collaborate with interdisciplinary team to promote resident well‑being and quality of life.
