Job summary

Silver Memories Health Care is seeking a dedicated Registered Nurse or Licensed Practical Nurse to join as a Registered Nurse Assessment Coordinator (RNAC / MDS Coordinator). This part-time role offers growth opportunities into full-time responsibilities, with potential expansion to Infection Prevention and Wound Nurse duties based on capacity.

Responsibilities include coordinating assessments, completing MDS and CAAs, and collaborating with interdisciplinary teams to ensure compliant care

Qualifications

• Current RN or LPN license in the State of Indiana.
• Minimum of one year of MDS experience in a skilled nursing or long-term care setting.
• Working knowledge of MDS processes, RAI guidelines, CAAs, care planning, and Medicare/Medicaid reimbursement.

Responsibilities

• Conduct resident assessments according to schedules and changes in condition.
• Complete MDS assessments, CAAs, care plans, and documentation per regulations.
• Develop, update, and monitor individualized resident care plans with the interdisciplinary team.
• Coordinate with Nursing, Therapy, Social Services, Dietary, and Activities for timely assessments and planning.
• Observe, assess, and communicate changes in resident condition.
• Prepare and maintain the MDS schedule and transmit data timely.
• Maintain documentation to support quality measures and regulatory compliance.

Silver Memories Health Care is seeking a dedicated and detail‑oriented Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team as a Registered Nurse Assessment Coordinator (RNAC / MDS Coordinator). This part‑time position is ideal for an experienced MDS professional looking for flexibility while making a meaningful impact on resident care and regulatory compliance.

For candidates seeking a full‑time opportunity, this role may be expanded to include Infection Prevention (IP) and/or Wound Nurse responsibilities based on experience and qualifications.

Your Role & Responsibilities

As the RNAC / MDS Coordinator, you will:

• Conduct resident assessments according to established schedules and in response to changes in condition, hospital readmissions, and other significant clinical events.

• Complete MDS assessments, Care Area Assessments (CAAs), care plans, and supporting documentation in compliance with federal and state regulations.

• Develop, update, and monitor individualized resident care plans in collaboration with the interdisciplinary team.

• Coordinate with Nursing, Therapy, Social Services, Dietary, and Activities to ensure timely completion of assessments and care planning.

• Accurately observe, assess, and communicate changes in resident condition.

• Prepare and maintain the MDS schedule while ensuring timely transmission of MDS data.

• Maintain accurate documentation and support facility quality measures and regulatory compliance.

For candidates interested in a full‑time opportunity, additional responsibilities may include:

• Serving as the facility's Infection Preventionist (IP) by assisting with infection surveillance, prevention initiatives, staff education, and regulatory compliance.

• Providing Wound Nurse services, including wound assessments, treatment coordination, documentation, physician communication, and monitoring resident progress.

What We're Looking For

• Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the State of Indiana.

• Minimum of one (1) year of prior MDS experience in a skilled nursing or long‑term care setting.

• Working knowledge of MDS processes, RAI guidelines, CAAs, care planning, and Medicare/Medicaid reimbursement.

• Experience with Infection Prevention and/or wound care is preferred for candidates interested in expanding to a full‑time role but is not required.

• Strong organizational, communication, and time management skills.

• Ability to work independently while collaborating effectively with the interdisciplinary team.
