Birchwood Rehabilitation and Healthcare Center is now seeking a dynamic, full-time RNAC.

Birchwood Rehabilitation and Healthcare Center is a 121-bed skilled nursing facility with private and semi-private rooms located in Nanticoke, PA. Qualifications
  • Licensed RN in the state of PA
  • 2 years RNAC experience in a long-term care setting
The Registered Nurse Assessment Coordinator (RNAC) would:
  • Ensure accuracy of all sections of Multiple Data Set (MDS) coding to maximize company reimbursement consistent with the levels of treatment delivered and required for appropriate patient/resident care (CMI).
  • Ensure accuracy of clinical paperwork (assessment and accompanying documentation) to maximize company reimbursement and ensure appropriate care.
  • Serve as a primary trainer to the nursing department to ensure documentation is accurate, timely, and thorough, requiring high levels of in-person interfacing with residents and staff.
  • Act as a key member of the nursing leadership team to clinically manage the unit and maximize patient-centered outcomes, including participation in leadership standups and interdisciplinary team collaboration.
  • Manage case work with managed care to maximize therapy and nursing services provided to residents.
  • Manage clinical and financial records and disseminate them to appropriate payers.
  • Participate in care conferences with social workers.
  • Perform other duties as required.
