Wesley Enhanced Living in Philadelphia, PA, is seeking a part-time Registered Nurse Assessment Coordinator (RNAC) to perform initial assessments and periodic re-evaluations of residents in our Skilled Nursing facility.
The RNAC will lead interdisciplinary care conferences, update care plans, coordinate case management, and ensure accurate MDS coding and clinical documentation. Requires PA RN license, OBRA knowledge, 2+ years' experience; AANAC preferred; CPR current.

Position Summary:
The Registered Nurse Assessment Coordinator (RNAC) provides for the initial assessment and periodic re-evaluation of all residents admitted to Skilled Nursing. Conduct Interdisciplinary Team Care Conference and implement and update resident Plan of Care. This position is part time.
