Job summary

Elara Caring is seeking an After Hours/ On Call Registered Nurse to join a mission-driven team delivering care where patients live. You will assess patients, coordinate with hospice teams, and support families in home settings, with travel up to 50%.

The role reports to the Clinical Team Manager and offers comprehensive benefits. Qualifications include an RN license, 1 year of clinical care, and hospice/home health experience preferred.

Qualifications

• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing.
• Current State License as a Registered Nurse (RN).
• At least 1 year of clinical care experience.
• Experience in hospice or home health is preferred.
• Travel up to 50% is required.
• Ability to frequently lift, push, pull up to 50 pounds, including patient transfers.
• Valid driver’s license and reliable transportation.

Responsibilities

• Coordinate care with the hospice team and leadership.
• Assess hospice care patients and families; document needs.
• Assess patient needs and data on physical, psychological, social, and spiritual factors.
• Communicate significant findings to the Clinical Supervisor, physician, and other staff.
• Counsel and include patient, facility, and family in the Interdisciplinary Plan of Care.
• Teach hospice philosophy and symptom management to staff in other care settings.
• Provide bereavement support at time of death.

As an After Hours/ On Call Registered Nurse, you’ll contribute to our success in the following ways:

• Ensures that all activities performed align with the vision of Elara Caring's board of directors, executive team, and the leadership of the Hospice team.
• Assesses hospice care patients and families to identify the physical, psychosocial, and environmental needs of patients as evidenced by documentation, clinical records, Interdisciplinary Team reports, after hours reports, and on‑site evaluations.
• Assesses patient needs and obtains data on physical, psychological, social, and spiritual factors that may influence patient/family/caregiver health status.
• Communicates significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and/or other personnel involved with patient care.
• Counsels, instructs, and includes the patient, facility, and family in following the Interdisciplinary Plan of Care.
• Teaches hospice philosophy and pain and symptom management to staff in alternate care settings (nursing homes, hospital, assisted‑living facilities, etc.).
• Provides appropriate support at time of death and perform bereavement assessment.

What is Required?

• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
• Current State License as a Registered Nurse RN
• 1 year of experience in a clinical care setting
• Experience in a hospice or home health environment is preferred
• 50% travel required
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Valid driver's license and insurance and reliable transportation to perform job tasks

You will report to the Clinical Team Manager.
