Job summary

Elara Caring is seeking a Hospice RN - After Hours to cover Jackson and Washtenaw County. This on-call role supports home-based hospice care, requiring 50% travel and flexibility to respond after hours.

The RN will assess patient and family needs, coordinate with the Interdisciplinary Team, and provide education on hospice care and symptom management. You report to the Clinical Team Manager and join a collaborative care environment.

Qualifications

• Graduate of an accredited Nursing program.
• Current State RN license.
• 1 year of clinical care experience; hospice/home health preferred.

Responsibilities

• Assess hospice patients and families to identify physical, psychosocial, and environmental needs; document findings.
• Communicate significant changes in condition to Clinical Supervisor, physician, and care team.
• Counsel and educate patient, family, and facility on the Interdisciplinary Plan of Care.
• Teach hospice philosophy and pain/symptom management to staff in alternate care settings.
• Provide bereavement support at time of death and perform bereavement assessment.

As an Hospice RN - After Hours you’ll contribute to our success in the following ways:

• Ensures that all activities performed align with the vision of Elara Caring’s board of directors, executive team, and the leadership of the Hospice team.
• Assesses hospice care patients and families to identify the physical, psychosocial, and environmental needs of patients as evidenced by documentation, clinical records, Interdisciplinary Team reports, after hours reports, and on-site evaluations.
• Assesses patient needs and obtains data on physical, psychological, social, and spiritual factors that may influence patient/family/caregiver health status.
• Communicates significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and/or other personnel involved with patient care.
• Counsels, instructs, and includes the patient, facility, and family in following the Interdisciplinary Plan of Care.
• Teaches hospice philosophy and pain and symptom management to staff in alternate care settings (nursing homes, hospital, assisted-living facilities, etc.).
• Provides appropriate support at time of death and perform bereavement assessment.

What is Required?

• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
• Current State License as a Registered Nurse RN
• 1 year of experience in a clinical care setting
• Experience in a hospice or home health environment is preferred
• 50% travel required
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Valid driver’s license and insurance and reliable transportation to perform job tasks

You will report to the Clinical Team Manager.
