Alliance for Care at Home is seeking a Community Hospice Admission Nurse to be the first point of contact for clients as they enter hospice. You will coordinate admissions with intake, review referrals for eligibility, and schedule in-person admissions to ensure a personal, goal-driven experience.

The role requires strong clinical nursing skills, excellent communication, and collaboration with the interdisciplinary team to initiate hospice care and ensure smooth handoff to the case manager.

Qualifications

• Must have RN licensure recognized by the State of Vermont.
• Minimum of four years as Community Hospice Nurse or five years in home health nursing.
• Hospice and Palliative Care certification preferred.

We are looking for a community hospice admission nurse who will be the first point of contact for our clients and families as they enter onto hospice and will ensure that the experience is personal and individualized while honoring specific client goals.

The admission RN will provide comprehensive coordination of admissions beginning with collaboration with our intake department, reviewing referrals for eligibility and then will schedule and complete in person admissions.You will collaborate with the client and caregivers around hospice plan, while organizing and directing the initiation of hospice care along with the associated charting and care coordination before handing off to the RN case manager and assigned interdisciplinary team members.

Minimum Requirements:

- Minimum of four years of experience as Community Hospice Nurse with a strong job performance record, or minimum of five years of experience in home health nursing, or a combination of other education and experience from which comparable knowledge and skills are acquired.

- Certification in Hospice and Palliative Care preferred.

On-call: Not typically, variable dependent upon program needs.
