Job summary

University of Vermont Medical Center is seeking a Registered Nurse - Admissions for Community Hospice in Colchester, VT. You will be the initial contact for patients and families, coordinating admissions and ensuring a personalized hospice experience.

The admission RN will partner with intake, review referrals, schedule in-person admissions, and work with the care team to implement the hospice plan and initiate care with proper charting and coordination.

Qualifications

• Minimum of four years of experience as Community Hospice Nurse or five years in home health nursing.
• Strong job performance record in hospice or home health settings.
• Certification in Hospice and Palliative Care preferred.
