Ambercare Hospice, part of the Addus HomeCare family, is seeking a compassionate Registered Nurse Admissions for Espanola, NM. This role includes pre-admission evaluations, education of patients and families, and coordinating admission processes to ensure high-quality home health care.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Must possess current CPR Certification.
• 1 year of Hospice experience, preferred.
• Must possess the ability to make independent decisions when circumstances warrant.
• Must possess the ability to deal tactfully with patients, family members, visitors, agency personnel, and the general public.

Responsibilities

• Perform pre-admission evaluations for the appropriateness of services.
• Initiates the admission process by educating and informing patients/families of its services.
• Obtains informed consent.
• Provides nursing assessment, initial plan of care, and intervention(s) that optimize patient and family-directed comfort and quality of life.
