Job summary

Ambercare Hospice in Espanola, NM is seeking a compassionate PRN Registered Nurse Case Manager to provide expert clinical care and support to patients and families. You will assess needs, determine eligibility, admit and plan care, implement and evaluate the nursing process, and make regular visits to re-assess and adjust the plan.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Must possess current CPR, First Aid Certification.
• Must be knowledgeable of quality assessment and assurance procedures.
• Valid driver’s license and proof of insurance is required.

Responsibilities

• Assume primary responsibility for patient and family caseload, including assessment, eligibility, admission processing, planning, implementation and evaluation of the nursing process.
• Obtain data and assess information from patient/family factors that impact needs and coordinate with the interdisciplinary team.
• Initiate communication with the attending physician and interdisciplinary team for coordinating optimal care.
• Provide regular visits to re-assess the patient's needs and update the plan of care.
• Perform clinical pronouncement of death and provide support to the family.
• Maintain accurate patient records, charts, progress notes, reports and care plans.
• Supervise and document delivery of care by the Home Health Aide as required by guidelines.

What You’ll Do:

• Obtain data and assess necessary information from patient physical, psychological, social, and spiritual factors that may impact the patient and family's needs and initiate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician, interdisciplinary team, and other agencies for the purpose of coordinating optimal care
• Provide regular visits to re-assess the patient's needs and update the plan of care
• Perform clinical pronouncement of death and provide support to the family
• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely
• Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines
