Job summary

Ambercare Hospice in Truth or Consequences, NM is seeking a compassionate Registered Nurse PRN to join our recently reorganized team. You’ll provide holistic patient care in home health settings, coordinating with an interdisciplinary team and supporting families during life’s important moments.

You must be a graduate of an accredited RN program, hold a current NM RN license, CPR and First Aid, and a valid driver’s license.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Must possess current CPR, First Aid Certification.
• Valid driver’s license and proof of insurance is required.

Responsibilities

• Assume primary responsibility for patient and family caseload, including assessment, eligibility, admission processing, planning, implementation, and evaluation of the nursing process.
• Obtain data from patient and family and coordinate with interdisciplinary team to address needs.
• Initiate communication with attending physician and other agencies to coordinate care.
• Provide regular home visits to reassess needs and update the plan of care.
• Perform clinical pronouncement of death and provide family support.
• Maintain accurate patient records, charts, and care plans; document delivery of care.
• Supervise and document delivery of care by Home Health Aide as required by guidelines.

What Youll Do

• Assume primary responsibility for patient and family caseload, including the assessment, determining eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process
• Obtain data and assess necessary information from patient physical, psychological, social, and spiritual factors that may impact the patient and family7s needs and initiate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician, interdisciplinary team, and other agencies for the purpose of coordinating optimal care
• Provide regular visits to re-assess the patient7s needs and update the plan of care
• Perform clinical pronouncement of death and provide support to the family
• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely
• Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines

Qualifications

• Graduate from an accredited registered nursing program
• Licensed as a registered nurse in the state of practice
• Must possess current CPR, First Aid Certification
• Must be knowledgeable of quality assessment and assurance procedures
• Valid driver s license and proof of insurance is required
