Job summary

The Care Team Hospice in Youngstown, OH is seeking a compassionate Hospice RN Case Manager to provide skilled nursing care, assessments, and symptom management for patients at the end of life. You’ll coordinate care plans, collaborate with the interdisciplinary team, and support families with education and guidance in line with physician orders.

Your role emphasizes timely visits, accurate documentation in Homecare Homebase, and after-hours coverage to ensure exceptional patient-centered care

Qualifications

• Active RN license required.
• Hospice nursing experience preferred.
• Experience with Homecare Homebase (HCHB) preferred.
• Strong assessment, communication, and care coordination skills.

Responsibilities

• Provide skilled nursing care, assessments, and symptom management in accordance with hospice standards, physician orders, and individualized plans of care.
• Complete scheduled visits, admissions, and clinical interventions while maintaining accurate and timely documentation in Homecare Homebase (HCHB).
• Respond promptly to patient, family, and caregiver needs, including after-hours and on-call situations, to ensure exceptional patient-centered care.
• Collaborate with physicians, caregivers, and interdisciplinary team members to coordinate care and support positive patient outcomes.
• Educate patients, families, and caregivers on symptom management, disease progression, medications, and end-of-life care while providing compassionate support.
