**Job Summary:**

JOB SUMMARY

Vanderbilt Heart and Vascular Institute knows how to care for your unique heart. Our team includes some of the top heart and vascular specialists in the nation. They can personalize a treatment plan just for you — whether it’s simply a change in your diet and exercise, medication, a catheter procedure, surgery or mechanical support.

Assists in developing and meeting key Pillar outcomes and system improvement goals, including financial, satisfaction, and clinical metrics, as part of the care coordination model. This role aims to improve outcomes by reducing hospital readmissions and coordinating care episodes for a defined patient population or disease process. Responsibilities include identifying appropriate patients, engaging patients and families in self-care, facilitating warm handovers, conducting patient education using the teach-back method, and helping patients navigate the healthcare system.

Hours for this clinic are from 7:30 am to 5 pm.

**KEY RESPONSIBILITIES:**

  1. Coordinate evaluation processes for the defined patient population, collaborating with team members to ensure completion of all required information.
  2. Support patient access by acting as a liaison between referring providers and Vanderbilt providers.
  3. Interact routinely with the clinical team to develop care plans from hospitalization through the care episode.
  4. Manage processes related to pre-admission and post-discharge care transitions, establishing relationships with providers and agencies to optimize care.
  5. Develop and disseminate patient education materials, including creating customized medication grids with input from pharmacists and team members.
  6. Assist with discharge and transition planning, actively engaging outpatient care providers and coordinating handovers with various care teams.
  7. Evaluate and improve care coordination practices, supporting protocol development based on evidence.

  • Evidence-Based Practice (Advanced): Recognize, implement, and evaluate practice changes based on research, demonstrating expertise in applying evidence-based practices.
  • RN Access Patient Education (Advanced): Deliver complex patient education, take a leadership role in challenging situations, and participate in professional forums.
  • Nursing Patient Assessment & Evaluation (Advanced): Conduct comprehensive assessments, mentor peers, and lead in complex patient care situations.
  • Trend Analysis (Intermediate): Recognize patterns, forecast demand, and analyze trends in various areas including quality and finance.
