Job summary

You will educate patients and families about conditions and care plans, ensuring safety and adherence to policies, while coordinating with case managers and other professionals to optimize outcomes and discharge planning.

Qualifications

• Associates degree or graduate from an accredited nursing program.
• BSN preferred or higher.
• NJ RN license required; BLS and trauma certifications may be required.

Responsibilities

• Provide patient-centered nursing care using the nursing process (assessment, diagnosis, planning, implementation, evaluation).
• Ensure patient safety and adhere to hospital policies and procedures.
• Educate patients and families about medical conditions and care plans.
• Collaborate with case managers and health-team members to optimize outcomes.
• Develop and document discharge plans to support ongoing recovery at home.
