Job summary

Apploi is seeking a Registered Nurse (RN) to provide direct nursing care at Lincoln Village Healthcare Center in Lincoln, Illinois. The RN will supervise LPNs and CNAs, ensure care aligns with policies and physicians' orders.

Ideal candidates hold an Illinois RN license, have a background in long-term care, and possess strong assessment, clinical, and communication skills. This is a non-exempt, full-time role with 12-hour shifts, and a pay rate of $50 per hour.

Qualifications

• Current, valid Illinois RN license in good standing.
• Graduate of an accredited nursing program (Associate or Bachelor’s degree).
• Prior experience in long-term care or geriatric nursing preferred.
• Strong assessment, clinical, and critical-thinking skills.
• Excellent communication, leadership, and organizational skills.

Responsibilities

• Assess residents’ health status and develop/update individualized care plans.
• Administer medications and treatments as prescribed, ensuring accuracy and timeliness.
• Monitor and respond to change in residents’ conditions, including notifying physicians and families as needed.
• Provide skilled nursing procedures such as wound care, catheter care, IV therapy, and tube feedings per physician orders.
• Complete and document nursing assessments in accordance with facility standards.
• Accurately chart medications, treatments, and resident care in the medical record.
• Participate in the completion of the MDS (Minimum Data Set) and other required assessments within regulatory timelines.
• Maintain accurate records for compliance with state and federal requirements.
• Provide supervision, direction, and evaluation of LPNs and CNAs.
• Conduct shift-to-shift handoff reports and ensure continuity of care.
• Participate in staff training, orientation, and performance evaluations.
• Promote teamwork and a positive work environment.
• Follow infection control, safety, and HIPAA guidelines at all times.
• Respond to emergencies and provide appropriate nursing interventions.
• Maintain a safe environment for residents, staff, and visitors.
• Comply with all facility, state, and federal regulations.
